Information for referrers

Thank you for referring to Wollongong Psychological Medicine. Clear referral information helps us assess suitability, urgency and the appropriate appointment pathway.

What to include in a referral

  • patient name, date of birth and contact details
  • reason for referral and the questions you would like addressed
  • relevant mental-health and medical history
  • current medicines and significant previous treatment
  • known allergies or adverse reactions
  • relevant investigation results
  • current risks and supports
  • other treating practitioners
  • service-specific information requested by the clinic

How referrals are reviewed

Referrals should be addressed to a named psychiatrist. If you are unsure who to address a referral to, please contact the clinic. All appointments are in person.

A referral is reviewed before an appointment is offered. Acceptance, urgency and clinician allocation remain subject to clinical review and capacity.

Referral requirements vary by service. General psychiatry may accept an appropriate medical referral; specialised programs may require a psychiatrist referral or an additional form. Each service page states the approved requirement.

Email is not a secure way to send personal health information. Only send information needed for your referral, with appropriate patient consent. If you have privacy concerns, call reception to discuss how to provide your documents.

Please ask your referrer to email your referral to reception, addressed to a named psychiatrist. All referrals are reviewed before an appointment is confirmed.

We are not an emergency or crisis service

Wollongong Psychological Medicine provides planned outpatient psychiatric care. Email, voicemail and booking requests are not monitored for immediate response.
